Application form instructions
  • Applicant must submit Original Bank deposit Slip along with application form to OTS.
  • Applicant must put Signature & Thumb Impression on Application Form.
  • Enrollment fee can be deposited at any bank or through easy paisa or banking app in any of the OTS scheduled banks.
  • In case fee is paid online, the print of receipt must be attached as a proof with application form.
  • Enrollment fee is non-refundable & non-transferable.
  • Application should reach OTS head office latest by last date of submission of Application Form.
